This is achieved by creating a very thin flap of corneal tissue that can be lifted, the laser energy applied, and then the flap can be readily replaced without the need for stitches.&lt;br /&gt;&lt;br /&gt;Dr. Spector’s training, experience and expertise in performing LASIK offers our patients an added sense of confidence in the safety, efficacy and predictability of having LASIK in Connecticut at Spector Eye Care in Norwalk &amp;amp; Stamford.&lt;br /&gt;&lt;br /&gt;What to Expect During Your LASIK Procedure&lt;br /&gt;&lt;br /&gt;   To begin your LASIK procedure, Dr. Spector will instruct the staff to place a few eye drops in your eyes to numb the surface of your eye. This will make you quite comfortable during your treatment. You will be seated in a comfortable and relaxed waiting area while the drops have a chance to begin working.&lt;br /&gt;&lt;br /&gt;   After the drops have had a chance to take effect, a staff member will escort you into testing area where a final measurement-called a wavefront refraction-will be reviewed. Next, a nurse will accompany you into the Excimer Laser suite and help you to position yourself comfortably on the moveable “bedlike” chair that is actually part of the laser system. Dr. Spector will gently place a small instrument between your eyelids to help you remember to keep them open during your LASIK surgery.&lt;br /&gt;&lt;br /&gt;   Dr. Spector will then create a very thin layer, which is a “flap” of clear tissue in the center of the cornea. He will then carefully fold the flap over and allow the inner layer of the cornea to be visible so that the laser energy can be applied. The actual laser application can take from 10-90 seconds depending on the amount of correction that you require.&lt;br /&gt;&lt;br /&gt;   You will be asked to look at a small light and to try and hold your eyes steady. There is no need to be concerned about moving your eyes during your treatment as the laser will actually “scan” the position of your eyes to identify the exact position of your eye and will then “track” and adjust its position in order to keep perfectly aligned with your eye movement.&lt;br /&gt;&lt;br /&gt;top&lt;br /&gt;&lt;br /&gt;   Based on your examination, your prescription and other measurements and calculations collected at your LASIK consultation, Dr. Spector will instruct the staff to program certain information into a computer that will monitor and control the actual application of the laser. Upon completion of the laser application, Dr. Spector will have changed the curvature of your cornea so that it will be the shape necessary to correct your vision.&lt;br /&gt;&lt;br /&gt;   Dr. Spector will then gently replace the “flap” back into its original position. Since the cornea is able to “stick” to itself, there is no need to use sutures or “stitches” to hold it in place.&lt;br /&gt;&lt;br /&gt;   LASIK surgery is quite “patient friendly”. The nature of the LASIK procedure is that it permits the healing and visual recovery process to be very quick and predictable for most patients. For a few days after your LASIK Eye Surgery, you will need to be careful not to rub or bump your eyes too much. It is expected that you will have a mild “sandy” or “gritty” sensation during the first day or so after your LASIK surgery. is an important cornerstone of clinical services at NCCS. Almost all tumours require surgical expertise to obtain tissues for diagnosis, and first line treatment for nearly all solid tumours is surgical resection.&lt;br /&gt;&lt;br /&gt;Staffing&lt;br /&gt;&lt;br /&gt;There are seven senior consultants, three consultants, two associate consultants, four registrars and one resident physician in the department. The department hosts visiting surgeons from government, restructured and private hospitals as well as other specialist centres.&lt;br /&gt;&lt;br /&gt;Surgical Subspecialties&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Breast Surgery&lt;br /&gt;Colorectal Surgery&lt;br /&gt;Gynaecology Surgery&lt;br /&gt;Head and Neck Surgery&lt;br /&gt;Hepatobiliary and Pancreatic Surgery&lt;br /&gt;Thoracic Surgery&lt;br /&gt;Upper Gastrointestinal Surgery&lt;br /&gt;Urological Surgery&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Surgical Procedures It is rapidly establishing itself as a regional referral unit for complex cancer surgery:&lt;br /&gt;&lt;br /&gt;Sentinel lymph node biopsy for breast cancer surgery&lt;br /&gt;Neoadjuvant chemotherapy and surgery for initially inoperable metastatic colorectal cancer to the liver&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Cancer Care in the Community&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;Care of cancer patients extends beyond oncology centres into the community where primary care is best delivered by family physicians. NCCS places a high priority on developing good working partnerships with our colleagues in the community. As part of our team based multidisciplinary care, each case would be discussed by oncologists at our centre as far as possible. Our discussion bulletins capture select case discussion from these tumour ward meetings and grand ward rounds. Should you have further questions that require input from our oncologists, we have an ‘ Ask the Experts 'column.&lt;br /&gt;Training Programmes&lt;br /&gt;&lt;br /&gt;The Department hosts two to four overseas fellows/ year keen to learn complex surgical oncology procedures.&lt;br /&gt;&lt;br /&gt;Research&lt;br /&gt;&lt;br /&gt;Singapore has successfully developed novel cancer diagnostics which have enabled their doctors to make early detection of certain types of cancers. This is done by adopting physical science techniques as in photonics imaging and bio-imaging techniques. Not known to many, the Centre is the leader in Singapore in pioneering non-invasive optical methods of cancer diagnosis. The techniques adopted allow what is not normally visible to the naked eye to be detected by optical imaging.&lt;br /&gt;Video-assisted minimally invasive surgery has revolutionised the care of patients with either benign or malignant chest problems. Certain surgical procedures can now be performed through smaller incisions, resulting in lesser post-operative pain nad faster recovery: at NCC, for example, we now routinely perform biopsies of the lung, pleura and mediastinal lymph nodes using this approach.&lt;br /&gt;&lt;br /&gt;Malignant liver tumours are either primary &lt;strong&gt;hepatocellular carcinomas&lt;/strong&gt; (hepatoma, HCC) or secondary metastatic tumours. The optimal treatment modelity for HCC is surgical resection. With metastatic disease to the liver, when the primary site of disease is the coloretum or a neurooendocrine malignancy, there may also be a palliative role for resection of the liver tumours. In some situations, however, liver resection is not possible.&lt;br /&gt;There are two types of lung cancer: non-small cell lung cancer and small-cell lung cancer. Non-small cell lung cancer is more common and it tends to grow and spread slowly. On the other hand, small-cell lung carcinoma is less common, and it tends to grow more quickly and spread to other organs.&lt;br /&gt;&lt;br /&gt;Increasing use of &lt;strong&gt;mammography and ultrasound examination&lt;/strong&gt; has led to a corresponding increase in the detection of non-palpable breast lesions.&lt;br /&gt;The case for discussion is a 45 year old patient with metastatic NSCLC, who presented with shortness of breath.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Customising treatment for cancer patients&lt;/strong&gt;&lt;br /&gt;Is the cure for cancer already in the market?  Surgical Retina</title><content type='html'>Who gets &lt;strong&gt;diabetic retinopathy&lt;/strong&gt;?(1 in 5 diabetics are affected)&lt;br /&gt;&lt;br /&gt;Diabetes mellitus damages small blood vessels throughout the body, affecting many organs e.g. the kidneys, nerves, brain and retina. Damage to the small blood vessels supplying the retina results in diabetic retinopathy, which is a major cause of blindness in many countries.&lt;br /&gt;&lt;br /&gt;All patients with diabetes are at risk of developing diabetic retinopathy. However, &lt;strong&gt;IDDM&lt;/strong&gt; patients are at greater risk due to earlier onset of disease. In a screening exercise for diabetic retinopathy in Singapore, 22% of patients were found to have retinopathy; and about half of these patients had severe, sight-threatening disease. This makes diabetic retinopathy a common condition in Singapore.&lt;br /&gt;&lt;br /&gt;Although retinopathy is strongly related to duration of diabetes, it starts earlier and is more severe in those with poorly controlled disease. When combined with other medical problems (e.g. high blood pressure and kidney disease), or even pregnancy, the risk of retinopathy is even higher. Hence, good control of blood sugar levels as well as other medical problems is of great importance in all diabetics.&lt;br /&gt;&lt;br /&gt;What are the different stages of &lt;strong&gt;diabetic retinopathy&lt;/strong&gt;?&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Diabetic retinopathy&lt;/strong&gt; is divided into 2 stages based on severity:&lt;br /&gt;&lt;strong&gt;1. Nonproliferative diabetic retinopathy (NPDR)&lt;br /&gt;2. Proliferative diabetic retinopathy (PDR)&lt;/strong&gt;&lt;br /&gt;&lt;br /&gt;In addition, the involvement of the centre of the retina, the macula, by fluid accumulation and swelling may accompany &lt;strong&gt;NPDR&lt;/strong&gt; and &lt;strong&gt;PDR&lt;/strong&gt;. This is called macular edema, and can cause early reduction in central vision, thereby impairing reading and sharp distance vision.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;1. Nonproliferative diabetic retinopathy (NPDR)&lt;/strong&gt;&lt;br /&gt;This is the early stage of diabetic retinopathy. The damaged retinal blood vessels leak fluid and blood into the surrounding retina. In addition, there is reduced blood circulation to the area normally supplied by these vessels, starving the retinal cells of oxygen and other nutrients.&lt;br /&gt;&lt;br /&gt;At this stage, unless the macula is involved, vision may still be good.&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;2. Proliferative diabetic retinopathy (PDR)&lt;br /&gt;&lt;/strong&gt;PDR is the advanced stage of retinopathy, and is a major cause of blindness in diabetics.&lt;br /&gt;&lt;br /&gt;This develops as retinal cells are increasingly starved of oxygen and the eye attempts to produce new blood vessels to compensate for this deficiency. Unfortunately, these blood vessels are abnormal and prone to breaking, which can result in bleeding into the eye. This blood results in poor vision and hinders treatment.&lt;br /&gt;&lt;br /&gt;In addition, scar tissue accompanies the growth of the abnormal blood vessels and can pull on the retina and cause retinal detachment.&lt;br /&gt;&lt;br /&gt;Abnormal vessels may also grow on the iris and angles and cause accumulation of fluid within the eye. The raised pressure within the eye as a result of this fluid accumulation is called “rubeotic glaucoma”.&lt;br /&gt;&lt;br /&gt;How will the eye doctor examine me for &lt;strong&gt;diabetic retinopathy&lt;/strong&gt;?&lt;br /&gt;&lt;br /&gt;Regular eye examination is important in all diabetics, including those with good control of sugar levels. Early diagnosis and treatment can significantly slow down the progression to sight-threatening disease.&lt;br /&gt;&lt;br /&gt;The examination will involve checking the iris for abnormal vessels, and measuring the pressure of the eye. The pupils will then be dilated so that the retina can be examined in detail using various lenses and instruments. Pupil dilatation may cause mild and temporary blurring of vision.&lt;br /&gt;&lt;br /&gt;In selected cases, a special test called fluorescein angiography may be done to help in diagnosis and treatment. The test involves injecting a fluorescent dye into a vein in the hand or forearm, and taking photos of the retina as the dye travels to the retinal blood vessels and lights up abnormal areas.&lt;br /&gt;&lt;br /&gt;How can &lt;strong&gt;diabetic retinopathy&lt;/strong&gt; be treated?&lt;br /&gt;&lt;br /&gt;Good control of blood sugar levels with medication and dietary restriction is the most important step in delaying the onset and slowing down the progression of diabetic retinopathy.&lt;br /&gt;&lt;br /&gt;1. L&lt;strong&gt;aser therapy &lt;/strong&gt;&lt;br /&gt;Once potentially sight-threatening retinopathy develops,&lt;strong&gt; laser therapy&lt;/strong&gt; will be required. Very often, more than one session will be required for complete treatment. This treatment is generally done on an outpatient basis under local anesthesia.&lt;br /&gt;&lt;br /&gt;It is important to understand that the benefits of laser therapy will not be immediately apparent. In fact, laser treatment may sometimes cause vision to worsen temporarily. Even where improvement in vision is expected, the effect may be seen only after weeks or months.&lt;br /&gt;&lt;br /&gt;Side effects of laser therapy may include shrinkage of the peripheral visual field, decreased night vision and reduced color vision. However, in advanced diabetic retinopathy, the benefits of laser therapy in reducing the risk of permanent and severe visual loss far outweigh the side effects.&lt;br /&gt;&lt;br /&gt;2. &lt;strong&gt;Surgery&lt;br /&gt;&lt;/strong&gt;In retinopathy too severe for laser therapy alone to be effective, or where end-stage complications (e.g. retinal detachment) have occurred, surgery may be the only treatment option. Surgery will include vitrectomy (removal of the jelly of the eye), and may be accompanied by other procedures like laser therapy at the time of surgery, surgical peeling of fibrous tissue and re-attachment of detached retina, etc. A gas bubble or silicon oil may be left in the eye after surgery. When significant, it may interfere with vision by blocking images from the superior field of vision. While the most common causes in adults is from prolonged contact lens use, injury or aging, it may also be seen as a anomaly at birth, which if not treated early may lead to permanent lazy eyes ( amblyopia). Treatment often involves a day-surgery procedure which not only enhances vision but also quality of life.&lt;br /&gt;&lt;br /&gt;Child with congenital ptosis before and after corrective surgery&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;2. &lt;strong&gt;Baggy eyes/Dermatochalasis.&lt;br /&gt;&lt;/strong&gt;&lt;br /&gt;This is commonly seen in older adults as an ageing change both in the upper and lower eyelids. To correct this, a plus (convex) lens is worn to focus light rays onto the retina, thereby allowing clear vision.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Astigmatism&lt;/strong&gt;&lt;br /&gt;Astigmatism occurs when the surface of the cornea is not uniform. This causes light rays to be brought to a distorted focus. Both near and distant objects are blurred. To correct this, a toric lens is worn to focus light rays onto the retina, thereby allowing clear vision. Astigmatism often occurs together with myopia or hyperopia.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;&lt;strong&gt;Presbyopia&lt;/strong&gt;&lt;br /&gt;Presbyopia occurs around the age of 40 when the focusing mechanism of the eye begins to become inefficient. Near objects begin to blur, and reading material have to be placed further and further away in order to be seen clearly.To correct this, reading glasses (plus power) are worn to help with near work. As the lens becomes less and less elastic over the years, higher power of reading glasses needs to be used.
